I initially started on Normal and after finding it a bit too easy, I restarted the campaign on Heroic. It's all kind of odd. Oh well, it's not like I hated the game or anything, I'm sure to play through it again. That may have been what happened. About the achievements for finishing on various difficulties, I know that if you beat it on Heroic, for example, you will also receive the achievements for beating it at all lower difficulties. Did you initially begin the Campaign on Normal, changing mid-stream to Heroic and then finishing it? That would mean you finished every level on Normal, but did not finish the earlier levels on Heroic, therefore you won't earn the Heroic completion. This happened to me when I joined some friends doing the Campaign on coop at Legendary difficulty after they had already completed a few levels which I had not. After we finished at Legendary difficulty I had to go back and complete the earlier levels on Legendary before I received the completion achievement at that level.
